During this time of COVID-19 we will be following the CDC guidelines for reopening. Please be aware we will be spacing our appointment times out to have less foot traffic in the office at one time and continue to adhere to social distancing rules per the CDC. All employees have been well and have been practicing social distancing. We are taking measures to keep you healthy through increased cleaning after each patient visit. We ask that you come to your appointment alone if possible. Please wear a facemask at all times while in our facility. If you do not have one, let the front desk know and one will be provided for you. When you arrive please immediately use the hand sanitizer at the front desk or go to the bathroom to wash your hands.

• If you have been in contact with anyone who has traveled in the U.S. or internationally in the past 2 weeks.
• If you have a fever or any respiratory symptoms such as cough or shortness of breath.
• If you have been around anyone who has had a fever or any respiratory symptoms such as cough or shortness of breath.
